Physical Submission Required — After submitting this form, print the attached PDF, obtain all required signatures, and mail in a sealed envelope addressed to the Department Adjutant. The envelope must be postmarked by February 15 each year.
The sealed envelope must have "AZ Marine of the Year" or "Associate of the Year" written in the lower left corner and contain only one recommendation.
