Complete all required fields below. Submitting will fill and email the official 2025 NOD PDF to all required recipients and send a confirmation copy to the Chaplain. Effective 11/20/2025 — no paper forms accepted.

⚠️ Important Notes

Do not send until all information is complete. Do not send multiple emails for the same NOD.

Detachment Paymaster: update the member's record in the MCL System Database. Paymasters no longer submit Transmittals for NODs (Regular or PLM Audits).

Chaplains: provide memorial acknowledgment using official MCL Sympathy cards. Cards should be sent within 60–90 days when possible. More than 4–5 months after the earthly call date is too late.
